Pregnancy creates significant changes throughout the musculoskeletal system as the body adapts to support a growing baby. As body weight increases and the abdomen expands, the center of gravity shifts forward, changing how forces are distributed through the spine and pelvis. These changes can increase demands on the lower back, hips, and surrounding muscles during standing, walking, sitting, and other everyday activities.
Hormonal changes also increase ligament flexibility in preparation for childbirth. Combined with changing posture and additional weight, this can affect pelvic and spinal stability and contribute to low back pain, pelvic discomfort, hip pain, muscle tension, or stiffness. The upper and lower body may compensate as well, which can make previously comfortable positions or activities more difficult as pregnancy progresses.
Pregnancy-related discomfort does not always require avoiding movement. Appropriate exercise, supportive sleeping positions, activity modifications, and conservative musculoskeletal care can help women remain active and comfortable. Chiropractic care during pregnancy can be adapted to the patient’s stage of pregnancy, symptoms, physical changes, and individual needs while complementing appropriate prenatal care.

Pregnancy can contribute to back pain because the body must adapt to increased weight, a changing center of gravity, and greater ligament flexibility. These changes alter the mechanical demands placed on the lumbar spine, pelvis, hips, and supporting muscles. Previous back problems, physical activity, prolonged positions, and individual biomechanics may also influence how much discomfort a person experiences.
Pregnancy-appropriate exercise can help maintain strength, mobility, and physical function while managing some types of musculoskeletal discomfort. Walking, swimming, and stationary cycling are common lower-impact activities, while individualized exercises may target the muscles supporting the spine, hips, and pelvis. Exercise selection should account for the stage of pregnancy, symptoms, previous activity level, and guidance from the patient’s prenatal healthcare provider.
Side sleeping with a pillow between the knees may help support the hips and pelvis and make pregnancy-related back discomfort more manageable. Pregnancy wedges or full-length body pillows can provide additional support for the abdomen, back, or legs as the body changes. The most appropriate position ultimately depends on comfort and any specific recommendations provided by your obstetric healthcare professional.
Chiropractic care can help manage pregnancy-related musculoskeletal discomfort by addressing relevant joint and muscle problems that affect comfortable movement. Treatment can be modified as pregnancy progresses and may be combined with therapeutic exercise, mobility work, and practical recommendations for everyday activities. Chiropractic care does not replace prenatal medical care and should complement the patient’s overall pregnancy care when appropriate.
At Cole Pain Therapy Group, our doctors provide individualized chiropractic care for pregnant patients experiencing low back, pelvic, hip, or other musculoskeletal discomfort. We consider spinal and pelvic joint function, mobility, muscular tension, activity limitations, and the physical changes occurring throughout pregnancy to guide care, which may include pregnancy-adapted chiropractic treatment and therapeutic exercise when appropriate. Care can also continue after childbirth to help address musculoskeletal demands associated with recovery, lifting, carrying, feeding, and caring for a baby.
