Neck or back stiffness can make it tempting to twist, stretch, or rotate your spine until you hear or feel a pop, especially when doing so seems to provide temporary relief. However, cracking your own neck or back is not the same as receiving a chiropractic adjustment, and the sound itself does not mean that a vertebra was out of place and has suddenly been put back into alignment. Joint sounds can occur during normal movement, so producing a pop does not necessarily tell you whether the source of recurring stiffness or discomfort has been addressed.
A chiropractic adjustment, also called spinal or joint manipulation, is a hands-on technique in which a trained clinician applies a controlled force to a specific joint when examination findings indicate that treatment is appropriate. The purpose is not simply to produce a popping sound, but to improve motion in a joint that is not moving normally and, when appropriate, help reduce pain and improve physical function. The technique, direction, positioning, and amount of force can be modified according to the area being treated, the patient’s symptoms, health history, physical condition, and individual needs.
One of the most important differences between self-cracking and professional chiropractic care is the clinical evaluation performed before treatment. Recurring neck or back pain and stiffness can have many potential causes, and spinal manipulation is not appropriate for every condition or every patient. Evaluating joint mobility, movement limitations, neurological findings, previous injuries, symptom patterns, and potential warning signs helps determine what may be contributing to the problem and whether chiropractic manipulation, another conservative treatment, or additional medical evaluation is the appropriate next step.

You may be able to make joints in your neck or back pop, but you cannot reliably perform the same type of targeted chiropractic adjustment on yourself. Self-cracking usually involves general twisting or stretching without determining which joint, if any, actually requires treatment. Repeatedly forcing an area to pop may also create movement in joints that are already mobile rather than addressing the source of persistent stiffness or discomfort.
A red flag is a symptom, medical-history detail, or examination finding that may indicate a potentially serious condition requiring further evaluation before chiropractic treatment. Examples include significant recent trauma, unexplained severe or progressively worsening pain, progressive weakness or numbness, changes in bowel or bladder control, fever associated with spinal pain, or other findings suggesting that symptoms may not be caused by a routine musculoskeletal problem.
Neck manipulation can be appropriate for carefully selected patients, but it is not suitable for everyone and carries potential risks, including rare serious complications. A chiropractor should review your symptoms, medical history, and examination findings before determining whether cervical manipulation is appropriate. When it is not, other conservative approaches such as joint mobilization, manual therapy, or therapeutic exercise may be considered.
You should not try to forcefully “realign” your spine by twisting or cracking your back. A popping sound does not indicate that a spinal bone has moved back into its proper position. Gentle movement and stretching may temporarily help uncomplicated stiffness, but recurring, severe, or worsening symptoms should be evaluated to identify what is contributing to the problem and determine the appropriate treatment.
At Cole Pain Therapy Group, our doctors evaluate neck and back problems by considering joint function, mobility, neurological findings, previous injuries, and movement limitations rather than simply trying to make the spine pop. When chiropractic manipulation is appropriate, the technique and amount of force are selected for the individual and may be combined with manual therapy, mobility work, and therapeutic exercise to address relevant musculoskeletal limitations and help restore comfortable function.
