Temporomandibular joint dysfunction (TMD), also known as TMJ disorder, is one of the most common causes of orofacial pain, affecting an estimated 10% to 15% of the U.S. population. Orofacial pain refers to discomfort involving the jaw, face, mouth, ears, and surrounding head and neck. While many people associate TMJ disorders with jaw clicking or popping, symptoms often begin with muscle imbalance, abnormal jaw movement, or excessive strain on the jaw long before noticeable joint noises develop. Patients commonly describe the discomfort as a dull ache, facial pressure, painful tightness, or a sensation similar to “needles digging” into the face.
In many cases, TMD originates from dysfunction of the temporomandibular joint (TMJ), the surrounding muscles, or both. The masseter, temporalis, and other muscles responsible for chewing work together with the muscles of the neck and upper cervical spine to produce smooth jaw movement. When these muscles become tight, fatigued, or uncoordinated, increased stress is placed on the jaw joint, supporting ligaments, and surrounding soft tissues. Activities such as chewing, talking for long periods, yawning, clenching the jaw, grinding the teeth (bruxism), chewing gum, or biting into hard foods may increase this stress and worsen symptoms.
Jaw clicking, popping, or locking may occur when the joint or its cushioning disc does not move normally, but these signs are not required for a diagnosis of TMD. Many people experience significant jaw pain without any joint noises. Because the nerves supplying the jaw also communicate with structures in the head, face, ears, and neck, TMD may cause referred pain, leading to headaches, ear pain, tooth pain, facial pain, or neck discomfort that can easily be mistaken for other conditions. A comprehensive evaluation is essential for identifying the true source of your symptoms and developing the most appropriate treatment plan.

Persistent jaw pain or difficulty using your jaw should be professionally evaluated. Warning signs include persistent jaw or facial pain, limited jaw opening, the jaw shifting when opening or closing, difficulty chewing, unexplained headaches, ear pain or tooth pain without a dental cause, and symptoms that interfere with eating, speaking, or sleeping.
TMD commonly causes pain around the jaw joint along with problems during jaw movement. Many people experience facial pain, tenderness near the temporomandibular joint, headaches, neck pain, stiffness, pain while chewing, or discomfort when yawning. Some notice clicking or popping in the jaw, while others develop muscle pain without any joint noises.
Activities that overload the jaw muscles or temporomandibular joint commonly trigger TMD symptoms. Common triggers include chewing gum, eating hard or chewy foods, prolonged talking, yawning, jaw clenching, teeth grinding, poor posture, forward head posture, and emotional stress that increases muscle tension around the jaw and neck.
Mild cases may improve with rest and temporary activity modification, but persistent or recurring symptoms often require treatment. If the underlying muscle imbalance or joint dysfunction is not addressed, symptoms may continue or gradually worsen over time. Early evaluation can help restore normal jaw function and reduce the risk of long-term problems.
