Tension headaches, also known as tension-type headaches (TTH), are among the most common types of headaches, affecting an estimated 30% to 70% of people during their lifetime. Many people describe the pain as feeling like their head is “in a vice” or as though they are wearing a “tight headband.” Although tension headaches are extremely common, they are often misunderstood because several factors can contribute to their development, and they may occur alongside other headache disorders.
The pain is typically dull, pressing, or tightening rather than throbbing, usually affects both sides of the head, and is generally not worsened by routine physical activity. If your headache is accompanied by nausea or vomiting, another headache disorder, such as a migraine, may also be present. Tension headaches are most common in young adults, but they can affect people of all ages.
Because several headache disorders can produce similar symptoms, obtaining an accurate diagnosis is essential. Many people experience tension headaches along with migraines, cervicogenic headaches, or medication overuse headaches. Identifying the underlying cause is the first step toward developing an effective treatment plan that provides lasting relief rather than temporary symptom control.

Temporary relief may come from reducing muscle tension and allowing your body to relax. Resting in a quiet environment, staying hydrated, gently stretching the neck and shoulders, and using over-the-counter pain relievers as directed may help relieve symptoms. However, if headaches occur frequently or continue to return, it is important to identify the underlying cause rather than relying solely on temporary symptom relief.
Tension headaches may be triggered by physical or emotional factors that increase muscle tension. Common triggers include emotional stress, lack of sleep, muscle tension, poor posture, prolonged computer use, physical fatigue, and repetitive strain affecting the neck and shoulders. Tension headaches may occur episodically (fewer than 15 days per month) or become chronic (more than 15 days per month for at least six months). Because one headache type can have multiple causes, and individuals may experience more than one headache disorder at the same time, a comprehensive evaluation is important.
Gentle neck and shoulder stretches may temporarily reduce muscle tightness associated with tension headaches. Stretching should always be performed slowly and comfortably without forcing painful movements. Because every headache has different causes, it is best to receive a proper evaluation before beginning a stretching or exercise program for recurring headaches.
Maintaining a neutral head and neck position is often the most comfortable position for people with tension headaches. Resting with proper support for the head and neck while avoiding prolonged forward head posture or slouching may help reduce muscle tension. A tension headache may last anywhere from 30 minutes to seven days, depending on the individual and the underlying cause.
