Chronic low back pain can make everyday activities such as sitting, working, exercising, lifting, and sleeping more difficult. With treatment options ranging from chiropractic and medical care to therapeutic exercise, rehabilitation, physical therapy, and massage, knowing where to begin can feel overwhelming. There is no single treatment that works best for everyone. The right approach depends on the underlying condition, how long symptoms have been present, mobility and strength limitations, and the impact on daily function.
Research comparing conservative treatments helps illustrate why individualized care matters. A randomized controlled trial published in the Annals of Internal Medicine followed 401 adults with chronic low back pain and compared massage therapy with usual medical care. Usual care could include pain medications, anti-inflammatory drugs, muscle relaxants, back exercises, and education, while participants assigned to massage received approximately one hour of treatment each week for 10 weeks. Researchers evaluated outcomes at 10 weeks, six months, and one year.
Participants receiving massage experienced greater improvements in back-related function and symptom severity at 10 weeks, with some benefits continuing during later follow-ups. By one year, however, differences between the groups had narrowed. The findings do not establish massage as the best treatment for every person with chronic low back pain. Instead, they reinforce the importance of selecting care based on the individual’s diagnosis, functional limitations, response to treatment, and long-term goals.
For people experiencing persistent lower back pain, successful management often involves more than temporarily reducing discomfort. Improving mobility, rebuilding strength, staying appropriately active, and addressing physical factors contributing to repeated episodes can all play a role. A comprehensive evaluation can help determine which conservative options are appropriate and whether additional testing or referral is needed.

There is no single cure for lower back pain because many different conditions can produce similar symptoms. Depending on the diagnosis, treatment may involve appropriate exercise, rehabilitation, chiropractic care, physical therapy, massage, medication, or a combination of approaches. The goal is to identify what is contributing to the problem and choose care that improves function while supporting long-term recovery.
Chronic back pain in men can have many possible causes, including muscle or soft-tissue injuries, repetitive physical stress, disc problems, previous injuries, and age-related spinal changes. Occupational demands, frequent lifting, prolonged sitting, and certain activities may also place additional stress on the lower back. Because the same symptoms can result from different conditions, an examination can help determine what is actually responsible.
Gentle movement, temporarily modifying activities that clearly worsen symptoms, appropriate exercises, and heat or cold may help manage some herniated disc symptoms at home. Prolonged bed rest is generally discouraged because remaining appropriately active can help preserve movement and function. Seek prompt medical care for new or worsening leg weakness, numbness around the groin or saddle area, or changes in bowel or bladder control.
The most effective approach depends on the cause of the pain and the individual’s needs. The research discussed above found greater short-term improvements with massage than usual medical care, but those differences became smaller over time. For many people, long-term management involves an individualized combination of appropriate treatment, progressive exercise, and strategies that improve the ability to perform everyday activities.
