Sciatica: Definition, Causes, and the Best Treatment

Sciatica is one of the most common causes of pain that travels from the lower back into the leg. Rather than being a diagnosis itself, sciatica is a symptom that occurs when the sciatic nerve becomes irritated, inflamed, or compressed. People with sciatica often experience pain that begins in the lower back or buttock and radiates down the back or side of the thigh, sometimes extending into the calf, ankle, or foot.

The sciatic nerve is the largest and longest nerve in the human body, extending from the lower spine through the buttocks and down each leg. Because it supplies sensation and muscle function to much of the lower extremity, irritation of this nerve can cause more than pain. Many people also experience numbness, tingling, muscle weakness, or difficulty walking, standing, or sitting comfortably for extended periods.

Sciatica is most commonly caused by conditions affecting the lumbar spine, including herniated discs, bulging discs, lumbar spinal stenosis, degenerative disc disease, osteoarthritis, and piriformis syndrome. Less commonly, muscle spasms, spondylolisthesis, or other conditions affecting the lower back or pelvis may also contribute to sciatic nerve irritation. Because several disorders can produce nearly identical symptoms, identifying the underlying cause is the most important step in choosing the most effective treatment.

Most cases of sciatica improve with conservative care, especially when treatment begins early. However, severe or progressive symptoms, particularly significant leg weakness or loss of bowel or bladder control, require immediate medical evaluation because they may indicate a medical emergency.

What causes sciatic pain?

Sciatic pain occurs when the sciatic nerve becomes irritated or compressed. The most common causes include herniated discs, bulging discs, lumbar spinal stenosis, degenerative arthritis, piriformis syndrome, muscle spasms, and other conditions affecting the lower back. Because several disorders can produce similar symptoms, identifying the underlying cause is essential before beginning treatment.

What does sciatica feel like?

Sciatica usually causes pain that radiates from the lower back or buttock into the leg. Many people describe the pain as sharp, burning, shooting, or electric-like, while others experience aching discomfort. Tingling, numbness, muscle weakness, or changes in sensation may also occur, and symptoms often worsen with prolonged sitting, standing, coughing, or certain movements.

How can you relieve sciatic pain?

The best way to relieve sciatic pain is to treat the condition causing the nerve irritation. Conservative treatment may include chiropractic care, therapeutic exercise, manual therapy, rehabilitation, and activity modification to reduce pressure on the sciatic nerve, improve mobility, and support recovery. Because sciatica has many possible causes, treatment should always be based on an accurate diagnosis rather than symptom relief alone.

Will sciatica pain go away?

Many cases of sciatica improve without surgery when treated conservatively. Recovery depends on the underlying cause and the severity of nerve irritation. Symptoms that persist, worsen, or are accompanied by increasing weakness, numbness, or changes in bowel or bladder function should be evaluated promptly by a healthcare professional.

At Cole Pain Therapy Group, we begin by identifying the underlying cause of your sciatica, whether it’s related to a herniated disc, spinal stenosis, piriformis syndrome, or another musculoskeletal condition.
