Temporomandibular joint (TMJ) pain can affect much more than the jaw. Discomfort may extend into the face, ears, head, neck, or shoulders and make everyday activities such as chewing, speaking, biting, and yawning uncomfortable. These symptoms are often associated with temporomandibular disorders (TMD), a group of conditions affecting the jaw joints, surrounding muscles, and other structures responsible for normal jaw movement.
Several problems can contribute to TMD. Myofascial pain may develop when the muscles controlling the jaw become tight, overworked, or irritated. Trauma can cause swelling, muscle spasms, joint sprains, or displacement of the small disc that helps the TMJ move smoothly. Degenerative conditions such as osteoarthritis may contribute to stiffness and reduced mobility. Stress can also aggravate an existing problem by increasing muscular tension and habits such as teeth grinding or clenching.
Common warning signs include jaw or facial tenderness, ear pain or fullness, headaches, clicking or popping, restricted mouth opening, and difficulty chewing. Some people may also experience discomfort extending into the neck or shoulders. Because jaw symptoms can overlap with dental problems, headaches, ear conditions, and other musculoskeletal disorders, identifying the actual source is an important part of choosing appropriate care.
A comprehensive evaluation typically looks at how the jaw opens and closes, joint mobility, muscle tenderness, and related movement patterns. When clinically appropriate, imaging may provide additional information. X-rays can help assess bone structures, MRI can evaluate soft tissues and disc position, and CT imaging can provide a more detailed view of the joint’s bony anatomy.

TMJ pain can result from problems affecting the jaw joint, surrounding muscles, or the disc within the joint. Overuse, trauma, osteoarthritis, altered joint mechanics, and myofascial dysfunction are possible contributors. Grinding or clenching the teeth may place additional stress on the area and aggravate an existing condition.
TMJ problems can cause jaw tenderness, ear discomfort or fullness, headaches, clicking or popping, limited mouth opening, and difficulty chewing or yawning. Symptoms vary depending on which tissues are involved, and not everyone with TMD experiences joint noises. Persistent pain, locking, or noticeable changes in normal movement should be evaluated.
The best treatment depends on what is responsible for the problem. Conservative options may include manual therapy, massage, targeted exercises, rehabilitation, and modifications to activities that aggravate the jaw. Heat may be useful for tight muscles, while cold therapy may provide relief during acute irritation. When nighttime grinding or clenching is involved, a dentist may recommend an appropriate oral appliance.
TMJ pain is diagnosed by evaluating your symptoms, medical history, jaw movement, muscle tenderness, and joint function. Imaging is not necessary for every patient, but X-rays, MRI, or CT scans may be recommended when additional information about the bones, soft tissues, or joint disc is needed to clarify the diagnosis.
